山形県警察本部で申請できる方は、原則として山形県内に住民登録又は外国人登録をされている方です。これらを他県に登録されている方は、登録地を管轄する警察本部にお問い合わせ下さい。提出国、申請理由によっては、直ちに申請受理できない場合もありますので、事前に山形県警察本部鑑識課までお問い合わせ下さい。

Applying For a Police Certificate
The following is information on Police Certificates.
In principle, applicants who apply for a Police Certificate at the Yamagata Prefectural Police H.Q. should have resident registration in Yamagata Prefecture. (Foreign applicants should have a Certificate of Alien Registration issued in Yamagata.) If applicants reside in other prefecture, they should contact the Police H.Q. which has jurisdiction in their area.
Since, depending on the reason for the application and the country of submission, we may not be able to accept an applications immediately, applicants must call the Yamagata Prefectural Police H.Q. in advance.Phone: 023-626-0110 (Ext.4652)
1 Applicant should bring the following items.
(1) This information is for those who reside in Japan (Japanese and foreigner).
A) A resident's card register (Issued within 6 months) (Jumin-hyo)
If the applicant is a foreigner, in place of residen's card register, a Certificate on registered matters [Tourokugenpyou Kisaijikou Shoumeisho] (Issued within 6 months) will suffice.
B) Applicant's valid passport
C) Documents indicating why the applicant needs to submit a Police Certificate. (Documents requesting a certificate issued by the concerned organization or a completed application that requires a police certificate to be submitted with it.)
(2) Those who reside abroad (Japanese and foreigner) need different documents. Please contact us in advance.

* Application takes about 30 minutes.
* It takes about 5 days for a certificate to be issued.

4 Issuance
The applicant will be contacted when the Police Certificate is ready.
Please come to the office to pick up the certificate.
* Applicants appearing in person should bring their passport.
* Persons other than the applicant should bring ID, such as a driver's license or a
passport, and letter of attorney.
